Students entering a public Virginia elementary school for the first time are required to provide:
1) A comprehensive physical exam from a qualified medical provider, performed within 12 months before the date the student first enters public school.
2) Documentation of required immunizations. A child with incomplete immunizations may be admitted conditionally, with proof of at least one dose of the required immunizations and a written schedule for completing the remaining doses within 90 days. (Exceptions apply for those with religious or medical exemptions.)
